Mailing labels are available for purchase from
the Arkansas Occupational Therapy Association. These labels are
standard size white address labels sorted in zip code order containing
names and addresses for OTs, COTAs and OT students.
For $200, you may order labels for our current
members of OTs, COTAs, student OTs, and student COTAs. For $180,
you may order labels for our current members of OTs and COTAs
only.
Members of AROTA can request one FREE set
of mailing labels per year. Please email your request for free
labels to the AROTA mail box, office@arota.org
for processing. ALL OTHER requests for labels must be mailed
along with payment.
To request mailing labels, please complete
this form and send it along with your check to AROTA, P.O. Box 337, Bryant, AR 72089. Checks should be made payable
to Arkansas Occupational Therapy Association. Labels will be mailed
upon receipt of request and payment.
