Occupational Therapy applications and forms can now be accessed and printed from our website at:  www.armedicalboard.org

The application requests made via phone are being directed to this new procedure.  If it is absolutely necessary, I can still mail them via phone request, but we are trying desperately to get away from this old habit and get with the 21st century!

The actual (3) page applications request will link the requester to an email because we are logging a track of them, but they can be requested and mailed from here via the site.

The individual forms can be printed straight from the website.  The requester should follow the simple directions.

Also remember that we are now requiring the Verification of Employment Affiliations (employment verified from the previous 5 years), this is a new form that is on the site and may very well not be in some of the old applications that some facilities maintained for new applicants.
